孟庆合得知喜来乐治好了格格的病，厚着脸皮来王府找喜来乐套近乎，想与他联手经营，遭到喜郎中的奚落。为达到将喜来乐撵出京城的目的，王天和不怀好意地邀请喜来乐到太医院与众太医切磋医术。太医们个个神情傲慢，像审问犯人一样戏弄喜来乐。师徒二人不卑不亢，—一应对，太医们细细品味之后，连连赞扬。王天和见状，又提起喜来乐用一个鸡蛋诈骗二十两银子的旧事。喜来乐反唇相讥，当众揭露了王天和伙同孟庆合贩卖假药的勾当。王爷再次挽留喜来乐，答应他让吏部给他谋个五品的官衔。喜来乐执意要回沧州老家，仍说治好格格的病是“瞎猫撞上了死耗子”，言谈之间，喜来乐连称王天和是有真才实学的御医。
