晚清的沧州，虽不及京城繁华，每逢年节或集日也呈现出热闹的景象。这日，几个汉子抬一不省人事的男人四处求医，“一笑堂”药铺的老板喜来乐闻讯赶来，一针下去，生命垂危的男人便醒转过来，围观的百姓连连地称他为神医。喜郎中被人称为神医，自己却患有惧内的顽症——他念念不忘对门“食为天”饭馆赛西施亲手做的“铁狮子头”。不幸的是，他的这一嗜好让夫人胡氏察觉了。为了吃“铁狮子头”，喜郎中私藏了自己行医挣来的银两，又被十分爱他且醋意十足的胡氏如数索去。喜来乐与徒弟德福着实过了把瘾。为支付吃“铁狮子头”的费用，师徒俩使尽了浑身解数，无意之中得罪了在“食为天”恭候皇室御医王天和的沧州药商孟庆合，同时，也冲撞了康有为的弟子鲁正明。
