喜来乐一家以扔铜板的方式决定是否进宫治病，结果只得认命。珍妃有病，实际上是老佛爷打的伤痕，喜郎中怕治好珍妃的伤会得罪老佛爷，未开药方，只开了个菜单：蛋白绿豆羹，白切豆腐片，并请皇上派人到“食为天”饭馆去取，意在让赛西施也进宫见见皇上和娘娘。喜来乐的两道菜治好了珍妃娘娘的伤，皇上龙颜大悦，将自己穿的黄马褂赏赐给喜郎中。王天和借着为太后看病的机会，把喜来乐为珍妃治伤的事告诉了老佛爷，又在众太医面前拨弄是非，引起众太医不满，吵吵嚷嚷要辞职回乡。德福趁喜来乐不备，穿起黄马褂招摇过市，白吃白拿，引起了众人的纷纷议论，却又敢怒不敢言。
