幸免于难的喜来乐师徒二人由衷感谢瑞芯姑娘的救命之恩，从中也悟出了“大火烧醒一场梦”的道理。喜来乐坚持要回沧州老家，鲁正明尽力劝阻，道明大老爷们儿，宁可让人打死，也不能让人吓死，喜来乐茫然了。沧州药贩陈皮从京城返回沧州，将喜来乐师徒被大火烧死的误传告诉众人后，胡氏、赛西施哭得死去活来。沧州的“一笑堂”内专门设置了灵堂，并排摆放着两口棺材，焚香祭奠之时，德福来到了“一笑堂”，令众人惊恐万分，待德福道明原委后，一家人反悲为喜，马上要到京城内与喜来乐团聚。胡氏醋劲未减，刚刚奉命进入京城，便责问喜来乐是否在京城拈花惹草，喜郎中被弄得哭笑不得。喜来乐在北京开设的“一笑堂”终于开张了，喜郎中本不愿与身为皇室御医的王天和争个高下，王天和却不容不让。英国医生史密斯来京城的“一笑堂”观光，参观之后深有感触。
