“一笑堂”内，前来求治的病人络绎不绝，喜郎中师徒忙得不亦乐乎。一直未能生养的胡氏来到京城后，觉得自己的肚子渐大，还特别喜欢吃酸的，便洋洋得意起来，喜来乐为她诊脉后，让她先去去肚子里的油水。王天和嫉贤妒能，千方百计地排挤喜来乐和他的“一笑堂”。喜来乐师徒应史密斯之邀，来到英国医生开的诊所，史密斯向他们讲述了人体的骨骼、内脏等医学常识，使喜郎中对自己从事的医疗工作有了突破性的认识。他与史密斯认真切磋中医疗法，史密斯对中医的博大精神大为赞赏，高兴之余，将一套动手术的医疗器械赠送给喜来乐。
