陆判担心打赌会输给司徒判，于是劝旦切勿放浪作恶，旦反求陆判替妻换头，并发誓只要妻子变得漂亮，他一定会忠于妻子，循规蹈矩。陆答应之，但说要等到有合适者才可。数天后，小曼在上元节游十王殿时，被恶贼杨大年看上，掳至偏僻处欲强暴之，小曼奋力抵抗，淫贼错手杀了青青。碰巧旦目睹此事，竟然见死不救，见小曼死，随即找来陆判，谓有合意者。陆只好徇私枉法，取小曼头，替少容作移花接木。旦见妻子变成美女，大为欣喜，容初时极之抗拒，换头后更无法适应，皆因换头不换心，但见旦喜爱，只好接受，却始料不及，从此惹来不少狂蜂浪蝶，令容不胜其烦。小曼落到冥府，成了无头鬼，陆判愧疚，欲速速打发小曼尽快投胎，可是小曼刚烈不屈，要见阎王对质，陆怕小曼大闹冥府，急判小曼进入地狱受苦，掩饰其过。小曼无头尸被发现，其父凭臂上胎记认回尸体，白扬闻讯后伤心欲绝。
