天使疗养院闹中取静，封闭如牢，收容的精神病患各色人等。林默逃无去处，被某人按在地上，对方攥着剪刀，猛插进他的鼻孔，四周响起欢呼喝彩。三天前，林默接到来自天使疗养院的电话，有人举报天使疗养院倒卖器官。他决定以身犯险，查明此案。林默查证，疗养院死者皆是社会边缘人士，死因是意外和病亡，无一列外。林默以精神病人的身份进入到疗养院后，得知举报者为疯人院钢琴师，林默到达当晚目睹钢琴师缢死。林默根据气味，开启嗅觉记忆锁定护士长，谁知病人沙马公然指认，目睹林默勒死钢琴师。护士长认为林默病情加重，需要送至重症病区，接受特别治疗。经过数人缠斗后，在被电晕之前，杰克贾出现，林默眼里恢复了光彩。
