１９８９年、まだ臓器移植の法制化が進んでいない日本では、移植手術が行なわれるのは当分先であると考えられていた。ある日、地方都市にある湖水町の甦生記念病院に、外科医の当麻鉄彦（滝沢秀明）が赴任する。医療先進国のアメリカで研鑽を積んできた当麻は、「地方でも大学病院と同じレベルの治療が受けられるべき」という信念のもと、あらゆる難手術に挑み、みごとな技術で目の前の患者たちの命を救っていく。 また、多くの患者の命を救うため、自らがアメリカで学んだ肝移植手術の必要性を訴える当麻は、同じく肝移植実現に取り組む医師・実川剛（仲村トオル）と対面する。日本の肝移植に大きな役割を果たす２人の医師が出会った瞬間だった。しかし、病院は自らの保身を第一と考え、リスクが大きい肝移植は行なおうとはせず、さらに、病院内の権力絶対主義と古い慣習に塗り込められた医療体制が当麻たちの行く手を阻んでいく。
