1891年。重度のコカイン中毒に陥った名探偵シャーロック・ホームズ（ニコル・ウィリアムソン）は、宿敵モリアーティ（ローレンス・オリヴィエ）を倒す妄想に捕らわれていた。ホームズを案じた親友のワトソン博士（ロバート・デュヴァル）は、精神分析の創始者ジグムント・フロイド（アラン・アーキン）の手を借りるため、魔都ウィーンへホームズを連れ出す。フロイドによって正常に戻ったホームズは、令嬢の誘拐事件を追う……。
